What is Chronic Condition Management?

If you’re reading this, you are more likely than not to have a chronic condition. 6 in 10 Americans have a chronic illness; 4 in 10 have multiple illnesses (1). That means most of us have a chronic condition. By definition, chronic illnesses last for a long time. So it’s important to have a doctor you trust, who listens to your feedback and is committed to your care for the long haul.
Traditionally, doctors have managed chronic illness with the “compliance model.” “Compliance” is measured by patient obedience, ability to follow medical advice, and personal responsibility (2). To be considered compliant, you must follow all of your doctor’s recommendations, including medication and lifestyle changes.
Unfortunately, the compliance model does not leave much room for patient collaboration or input. Its numerous drawbacks include a lack of preventative care, lack of patient education, and unhelpful pressure placed on patients. Sadly, this often means you don’t get the time and attention you and your health deserve.
This rigidity can result in patients who are scared or ashamed to bring up issues in “complying” to their given treatment. This then leads to patients who stay silent about their struggle to follow treatment plans, which can cause diagnoses to be missed and health to worsen. If you aren’t comfortable being honest with your doctor, it’s practically impossible to get good healthcare.
When it comes to managing chronic conditions—like diabetes, heart disease, arthritis, and more—we need a better way. To truly improve medical outcomes and empower patients to get healthy, doctors must work with patients to find the treatment(s) that works best for them.
But how do you find care like that? Surely, you can’t find affordable, accessible, and legitimate doctors willing to actually collaborate with you when it comes to your treatment! But, oh, wouldn’t it be nice? What if you could get professional medical care that emphasizes patient collaboration, all from the comfort of your own home?
